    What is Lithotripsy? Advantages and Disadvantages of Lithotriptor ?

    What is Lithotripsy ?

     Lithotripsy is coupled with two words - litho (stone) and tripsy (break), implies - stone-breaking, yet, it is otherwise called ESWL (Extracorporial Shock Wave Lithotripsy). It is just a therapeutic technique for crushing or breaking the kidney stones in small fragraments with shock wave machine without making an operative incision. Parts involved with this procedure are kidney, urinary bladder, ureter only and not need to hospitalization because it done as outpatient basis.
    Most kidney stones are small enough to pass completely without treatment but many of patient who have bigger one (up to 20 mm) then treatment is necessary 
    and now a days  ESWL is an incredible choice of treatment because it  managed without surgical incision, has a great and positive aspect of this therapy. But a notable point is that this procedure is effective in up to 20 mm size stone and the treatment might not be effective in larger than 20 mm measured stones. ESWL works well with smaller than 20 mm stones dependably.


     Advantages of Lithotripsy  

    -It is a non-surgical treatment.

     -No hospitalization required since, it done on an out patient basis, so patient can go their home same day.

     -The recovery time is usually quite short. After treatment, the patient can get right up to walk simultaneously up, many individuals can completelly continue every day activities within one to two days.

    -Precautionary diets aren't needed, just drinking a plenty of water should be implemented, to help flushing out stone fragments.

    Disadvantages of Lithotripsy  


    - Anaesthesia – 

    It is a non-surgical and non- incisional technique although mild or moderate sedation may be required and it is completely relied upon patients' health condition and size of the stone and doctor's choices.

    - It is useful for small stones. Larger than 20 mm stones, may need surgical removal or other treatment.

     - Treatment may require a few sessions of procedure. After treatment, some patients may still have stone fragments inside their urinary tract that are too larger. These can be treated again either with lithotriptor or need other solutions to get rid off.

     -After ESWL,  patient might feel abdominal pain or bleeding from kidney, ureter etc. Analgesics (NSAID) and drinking a lot of normal water may soothe symptoms. Some times, unsuccess would be made with this therapy, that results in not break down of stones.

     - Some of 50-75% are observed to be free from stones within 90 days of ESWL 
    treatment. The highest success rates seem to be in those patients with smaller stones.

     CONTRAINDICATIONS - 

    -Pregnant lady 

    Shock wave may harm foetus, so , pregnant lady is not really a suitable candidate for lithotripsy procedure.

     Obesity - Over weighed people are contraindicated for this treatment also. 

    Intestinal flatulence –

    Intestinal gas may interfere with a Lithotripsy procedure. 

    - Patients with acute or chronic uti - urinary tract infection.

     - Patients having certain type of hard calcium and cystine stones, sometimes not break up with lithotripsy. 

    -Patients having a pacemaker-
     Cardiologist advise is needful prior to procedure. 

    -Patients with ureter or urethral stricture, which may prevent stone fragments from passing.

    -Patients who require quick and complete removal of stones.

    - Patients with bleeding disorder (haemophilia) or who have taking blood thinning agent (anticoagulant such as aspirin), need discontinuation of anticoagulant drug prior to procedure.

    -Damage to the area around the stone, fragrant or pieces of the stone blocking the flow of urine. 

    Complications  of  lithotripsy -

    Blockage in urine flow due to broken fragrant which might be sticky. Uretroscopic fragments removal may need in such sorts of conditions. 

    -UTI (Urinary Tract Infections).

    -Pain may occur due to stone fragments.

    Recurrence of stones - Reformation of kidney stones may seem in some of the cases and repeat treatment likewise required in that type of cases.

     - Tissue damages – 

    Some of the fragments become hone edges and may cause tissue injury and severe pain around stuck stone region.
